e-Waiver Form

Some events require that members complete a TRIP AGREEMENT AND RELEASE OF LIABILITY form, which we refer to as the Waiver form.   In the past we required the member to complete a form specifically for each event.

We’ve replaced it with an annual version.

Simply click the button below, fill out the form on your cell, notepad, laptop or desktop computer and click the Submit button and you’re good for the rest of the year !!!

e-Waiver Report

Event leaders need to access the information submitted by participants on the e-Waiver form

  • confirm that everyone has submitted an e-Waiver form
  • participants cell number
  • ICE information

Simply click the button below, fill out the Request form and a list of participants will be emailed to you.  As with the e-Waiver Form, this form as well as the email report can be done on your cell, notepad, laptop or desktop computer.
